We are currently hiring a Community Health Worker based in Framingham, MA.
The Community Health Worker (CHW) is a key member of the health care team, providing comprehensive and coordinated interventions to address the social determinants of health (SDOH) as part of a patient-centered medical home.
The CHW facilitates access to care and services by providing education and advocacy to assist patients in navigating the health care and social service systems.

Job
Summary:
Conduct psycho-social assessments with patients referred for Community Health Worker interventions in accordance with Health Center policies, procedures and protocols.
Provide health system education and advocacy to assist patients in navigating the health care system.
Provide basic education, screening, brief intervention and/or facilitate referrals to other health center or community-based resources in accordance with applicable health center protocols.

Job Requirements:
Minimum 2-years work or volunteer experience in human services or healthcare with demonstrated knowledge of or ability to work with and/or within the targeted community With limited exceptions approved by the supervisor in advance, a CHW is expected to have a valid U.
S.
Motor Vehicle License and reliable transportation to travel to/from outreach activities, as they may be assigned from time to time.
If the CHW will also be assigned to transport patients, the CHW must provide proof of state-mandated personal Motor Vehicle insurance.
Demonstrated technical experience with Microsoft Word, Excel (or similar databases), and Internet Explorer.
Excellent oral and written skills.
BLS Certification Our health center requires all employees to be fully vaccinated for COVID19 and Flu.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
